Home from Hospital Service

The Red Cross Home from Hospital Service provides trained Red Cross volunteers to help patients when they first go home from hospital as part of the hospitals discharge plan. Referrals are accepted from ward staff, relatives, care managers, patients, friends, doctors or anyone who feels the patient is in need.

The service is free of charge for a period of 4 weeks. A volunteer goes to visit the patient in his or her home for a maximum of two hours once or twice a week at a mutually convenient time. This would be arranged directly between the patient and the volunteer.

The support offered by the volunteers can smooth the process of settling back into a normal routine and enable people to regain their confidence and independence. The service includes:

  • assistance with shopping
  • collecting prescriptions
  • offering companionship
  • rebuilding confidence
